You MUST Bring the Following:
1. Shot records (Must be Current)
2. Birth Certificates (must be official state copy, not hospital copy)
3. Student's Social Security card
4. Parent driver's license or photo identification
5. Proof of residency (Current water, electric, or gas bill with the physical address required) No phone bills will be accepted.
6. If registering for Head Start or Pre-K program:
Proof of income is REQUIRED (Last paycheck stub or statement from the employer, W2, Income tax return). IF NO INCOME, proof of financial assistance from government agency (food stamp letter, SSI statement, TANF statement).
No partial registrations will be accepted. You must have all documents to register your child.
