current shot record

proof of residency - electric bill, phone bill, anything with a physical address

proof of income for Head Start or Pre-K

copy of drivers license

copy of child's social security card or recent income tax return with student’s SS#

copy of birth certificate ( original from the state; can't be from the hospital )
